Understanding healthcare options

Costa Rica offers public universal healthcare, a strong private system and hybrid approaches. Choose deliberately rather than discovering the system during an emergency.

In this chapter

The public system — the Caja Costarricense de Seguro Social, “la Caja” — provides universal coverage to residents who pay income-based contributions. Care is comprehensive but waits for non-urgent specialist care can be long. Legal residents are generally required to enrol.

Private care is widely used by expats: private clinics and hospitals in the Central Valley offer shorter waits and English-speaking staff at prices well below US levels. Many residents use both: the Caja for major and chronic care, private for speed and convenience.

  • Local clinics and pharmacies handle everyday needs in the Southern Zone.
  • Serious or specialist care usually means travelling to San José.
  • Private insurance plans exist locally and internationally; compare coverage for your age and conditions.
  • Pharmacies can provide many medications without a new prescription and offer basic medical advice.
